Please call your insurance company prior to your appointment. They can help you determine your copay, what portion of your deductible has been met, and what portion your insurance may cover.  
 

We bill your insurance as a courtesy. You are responsible for any costs not covered by insurance, including missed/late cancellations, paperwork fees, deductibles, etc.
